Why Does My Back Hurt When I Lie Down, and What Helps in Bed?

If your back hurts when you lie down, you are far from alone. Most of the time it is a stiff back meeting a flat mattress. A pillow under or between the knees often eases it within minutes. Two places to press lying down: the backs of your hands (Yaotongdian, EX-UE7) and the inner edge of your foot.

Quick answer

QuestionShort answer
Why does it hurt as soon as I lie down?Usually a stiff lower back that can't flatten onto the mattress, or one that tightens once you stop moving.
What helps first?Change the shape you lie in: a pillow under the knees on your back, between the knees on your side.
What can I press lying down? The backs of both hands, halfway between wrist and knuckles: Yaotongdian (EX-UE7). One to three minutes, moving your back gently.
What about the feet? Before bed, sitting: the back half of the inner arch, the lower back zone of the foot.
How hard?Hunt for the spot that stings. That's the one. Stay on it.
When is it not a back-muscle problem?Pain that is the same in every position, wakes you every night, or comes with fever, weight loss or weak legs. See your doctor.

Why your back hurts when you lie down

In the day, your back is held by movement. You shift, walk, lean, stand up. A stiff back can get through a day like that.

Lying down asks something different. Lie flat on your back and the curve of your lower back has to settle toward the mattress. If the muscles and joints there have stiffened over the years, they can't let go that far, so they pull. That pull is the ache you feel in the first minutes in bed.

Lie still for a while and a second thing happens. Muscles that were working all day stop moving and begin to set. When you turn over, the back that has set sends a sharp message.

Neither of these means you did something wrong. They mean your back is guarding. A guarding back needs a softer shape to lie in, and a few minutes of help before it lets go.

Positions that ease a back that hurts lying down

Try these before pressing anything. For many people the position does most of the work.

  1. On your back: a pillow under your knees. Bending the knees lets the lower back settle without stretching. A rolled towel under the small of the back helps some people too.
  2. On your side: a pillow between your knees. It stops the top leg from twisting your lower back. Bend both knees a little, as if sitting in a chair.
  3. On your front: a thin pillow under your hips, or better, roll onto your side. Lying face down arches the lower back.
  4. Getting into bed: sit on the edge, lie down onto your side, then lift both legs up together. Getting out, do the same in reverse: roll to your side, drop your legs over the edge, push up with your arms.
  5. The mattress: if it sags in the middle, your back sags with it. Try a firm board under it, or a different bed for a few nights. That tells you whether the mattress is part of the story.

Two places to press when your back hurts lying down

The backs of your hands — Yaotongdian (EX-UE7)

Yaotongdian (EX-UE7) pressure point for back pain when lying down: the back of an open hand with two red dots midway between wrist and knuckles, in the grooves between the bones.
Two spots on the back of the hand, halfway between the wrist crease and the knuckles.

Lie on your back with your knees bent, or with a pillow under them. Rest one hand on your chest, palm down.

Look at the back of that hand. Four long bones run from the wrist to the knuckles, with three grooves between them. Ignore the middle groove. Use the groove on the thumb side and the one on the little-finger side, halfway between the wrist crease and the knuckles.

Probe each groove with the thumb of your other hand until one spot stings. Rub it in small circles, one to three minutes. While you rub, rock your knees slowly a few inches to one side and back, and let your lower back move with them. Stay in the range that does not hurt. Then change hands.

In Vietnamese traditional medicine this pair is reached for when the lower back locks. You don't have to bend or reach to use it, which is why it suits a back that hurts lying down. More on the Yaotongdian (EX-UE7) points.

The inner edge of the foot (the lower back zone)

Do this one before bed, sitting on the edge of the bed with one foot across the other knee, inner edge facing you.

Find the small bony bump a thumb's width in front of and below the inner ankle. From there, the strip runs forward along the lower edge of the arch, two or three finger-widths long. In Vietnamese foot work, the inside edge of the foot follows the line of the spine. This back half of the arch matches the lower back.

Make three to five slow passes along it, about a minute per foot. The whole zone is on the lower back zone of the foot page.

How I press the points for back pain when lying down

I have studied Vietnamese acupressure for fifteen years. This is the way I was taught.

  1. Find the area. You don't need a pinhead. Get into the right neighbourhood using the bones and grooves above.
  2. Search for the tender spot. Move slowly around the area. One spot will feel a little sharper than the rest.
  3. Make small, slow circles on that spot. One minute at first, building to two or three over the next nights.

Hunt for the spot that stings. That's the one. Stay on it. Leave a spot alone if it still hurts after you lift off.

Which is harder for you: bending down to reach your feet, or thumbs that tire after a minute? If it is either, a tool can do the pressing. The Lower Back & Feet Kit has two pieces. A round-ended stick glides along the inner edge of the foot, so you don't have to fold down to it. A wooden stick sits in your palm and works the grooves on the back of the other hand while your thumbs rest. Your hands get tired. The tool doesn't.

It comes with press-along videos you follow from your chair. Unused tools can be returned within 30 days of delivery; email us first so we can approve it. No pressure, though. The positions and the pressing above work with your own hands.

When to see a doctor about back pain when lying down

Most back pain in bed is a stiff back. Some is a reason to see your doctor before trying anything at home. Please make that appointment if:

  • the pain is the same in every position, and nothing you change in bed makes a difference
  • it wakes you every night, or is worse at night than in the day, week after week
  • it is new, or getting worse
  • it comes with a fever, night sweats, or weight loss you can't explain
  • you have had cancer in the past
  • one or both legs feel weak, or numbness is spreading
  • you have trouble controlling your bladder or bowels, or numbness around the bottom — this one is urgent, call for help the same day

If you take blood thinners, are pregnant, or had back surgery recently, check with your doctor before pressing. For a medical view, read the NHS guide to back pain.

The honest timeline for back pain when lying down

A better position can help the same night. That is the quickest change you can make.

Pressing is slower. You may feel a spot ease within a few minutes. A back that stops protesting when you lie down usually takes longer: two to three weeks, about ten minutes each evening.

Research on acupressure for low back pain shows encouraging signs, but the studies are limited in quality. Some people notice very little. I would rather tell you that now.

What This Won't Do

This won't change what is happening inside the joints of your spine. It won't replace anything your doctor has given you; keep taking exactly what was prescribed.

What it gives you is a softer shape to lie in and ten minutes of something you can do yourself before the lamp goes off. Does your back also wake you in the small hours? Read lower back pain that wakes you at 3 a.m.. More pages for a sore back are on back pain relief.

Questions people ask about back pain when lying down

Why does my back hurt when I lie down but not when I stand? Standing and moving keep a stiff back loose. Lying flat asks the lower back to settle toward the mattress, and lying still lets the muscles set. A pillow under or between the knees usually helps.

What is the best sleeping position for lower back pain? On your side with a pillow between your knees, or on your back with a pillow under your knees. Lying on your front arches the lower back, so most people with a sore back do better avoiding it.

What can I press for back pain lying in bed? The backs of both hands, halfway between the wrist and the knuckles, in the grooves on the thumb side and the little-finger side: Yaotongdian (EX-UE7). Rub one to three minutes while rocking your knees gently.

Why is lower back pain at night a red flag? Most back pain at night is a stiff back, and it changes when you change position. Doctors take notice when it doesn't: pain that is the same in every position, wakes you every night, or comes with fever, weight loss, a past cancer, weak legs or bladder trouble. That pattern needs a doctor's check, not pressing.

Why does my lower back hurt when I sit up from lying down? Sitting straight up asks a back that has been still for hours to bend and carry your weight in one move. Roll onto your side first, let your legs drop over the edge of the bed, then push up with your arms. Your back comes up in one piece.

Why does the back of my knee hurt when I lie down? Often the knee has been held straight for a long time, and the tight back of the thigh pulls on it. A pillow under the knees helps both the knee and the lower back. I wrote more on knee pain at night.

How long before I notice a difference? A better position can help the first night. Pressing, if it helps you, usually shows in two to three weeks of a few minutes each evening.

I have spent fifteen years studying traditional Vietnamese medicine, and I write about things you can do for yourself at home, sitting down.

This is general wellness education, not medical advice. It is not a substitute for care from your doctor. If you have a health condition, are pregnant, or take medication, check with your doctor first.
