Lower Back Pain Wakes Me at 3 a.m.: What Can I Press, and Which Tool?

Lower back pain that wakes you at 3 a.m. is real, and the night is not the time to press. In the night: change position, breathe out slowly, stay warm. Before bed, sitting up, press four places: behind the knee (BL40), the outer ankle (BL60), the inner edge of the foot and the outer hand (SI3).

Why the lower back wakes you around 3 a.m.

In the day, you move. Movement keeps a stiff back from setting. At night you lie still for hours, and the muscles around the spine settle into one shape.

Then you roll over. A back that has set like glue sends a sharp message, and you wake. The house is quiet, so the ache feels louder than it did in the day.

It is not only your age, and you are not doing anything wrong. A back that has hurt for years is often a back on guard. The aim of this page is a calmer back at bedtime, and an easier way back to sleep.

At 3 a.m.: what to do instead of pressing

I press in the daytime and before bed. I don't press at 3 a.m. Searching for a tender spot is wake-up work, and the job at night is the opposite.

  1. Don't look at the clock. Knowing the time starts the sums in your head.
  2. Change the position that hurts. On your side, put a pillow between your knees. On your back, put a pillow under your knees.
  3. Breathe out longer than you breathe in. Slow, through the mouth. No counting needed.
  4. Check your feet. Cold feet make settling harder. Keep socks by the pillow.
  5. If you have to get up, sit on the edge of the bed for 30 seconds first. Feet flat, then stand slowly with one hand on the bed.

If your nights also include trips to the bathroom or a busy mind, I wrote a longer page on waking up in the middle of the night.

Before bed: four places to press for the lower back

Sit on the edge of your bed, or in a firm chair, feet flat. Do the four places in this order, then the other side. The whole round takes about ten minutes.

The back of the knee — Weizhong (BL40)

Weizhong (BL40) pressure point for lower back pain that wakes you at 3 a.m.: back view of two standing legs, red dot at the middle of the crease behind the right knee.
Look at the back of the knee where the skin creases; the spot sits in the middle, halfway across, right at the crease.

Bend your knee to about a right angle. Wrap both hands around it so your fingertips meet in the hollow behind it. Feel the firm cord on the outer side and the one on the inner side.

The spot is on the skin crease, halfway between them. Probe gently until one place feels tender. If you feel a pulse under your finger, move off it.

Use light circles only here, one to three minutes. Then the other leg.

The outer ankle — Kunlun (BL60)

Kunlun (BL60) pressure point for lower back pain that wakes you at 3 a.m.: side view of the outer ankle and foot, red dot in the hollow behind the outer ankle bone, in front of the heel tendon.
Find the round bump of the outer ankle bone, slide straight back toward the heel tendon, and probe the dip where the red dot sits.

Rest one foot on a low stool, or across the other knee. Find the highest point of the bone on the outside of your ankle. Then find the thick tendon at the back of the heel.

Slide a fingertip straight back from the ankle bone into the soft dip halfway to the tendon. Search it for the spot that stings, then make small circles for one to three minutes.

Skip this place if you are pregnant, if the ankle is freshly sprained, or over a bulging vein.

The inner edge of the foot (the lower back zone)

Keep the foot where it is, inner edge facing you. Find the small bony bump a thumb's width in front of and below the inner ankle. From there, the strip runs forward along the lower edge of the arch, two or three finger-widths long.

In Vietnamese foot work, the inside edge of the foot follows the line of the spine. This back half of the arch matches the lower back. Make three to five slow passes along it, about a minute per foot. More detail is on the lower back zone of the foot page, and on the foot point at the top of the arch.

The outer edge of the hand — Houxi (SI3)

Houxi (SI3) pressure point for lower back pain that wakes you at 3 a.m.: loosely closed fist seen from the little-finger side, red dot where the palm crease ends.
Close a loose fist. The spot is where the palm crease ends, at the outer edge of the hand.

Rest one hand in your lap and curl it into a loose fist. The palm crease nearest your fingers stands out. Follow it outward with your other thumb until it reaches the edge of the hand, behind the little-finger knuckle.

Probe that edge until one spot stings. Rub it in small circles, toward the bone, for one to three minutes. Then change hands. This is the easiest of the four on a night when bending is hard.

How I press the points for lower back pain that wakes you at 3 a.m.

I have studied Vietnamese acupressure for fifteen years. This is the way I was taught.

  1. Find the area. You don't need a pinhead. Get into the right neighbourhood using the bones and cords above.
  2. Search for the tender spot. Move slowly around the area. One spot will feel a little sharper than the rest.
  3. Make small, slow circles on that spot. One minute at first, building to two or three over the next nights.

Hunt for the spot that stings. That's the one. Stay on it. Leave a spot alone if it still hurts after you lift off.

Which part is hardest for you: bending down to your feet, or thumbs that tire after a minute? If it is either, a tool can do the pressing. The Lower Back & Feet Kit has two pieces. A round-ended stick glides along the inner edge of the foot and finds the ankle hollow. A wooden stick sits in your palm, so your whole hand holds it and your thumbs rest. Your hands get tired. The tool doesn't.

It is $37.99 with free shipping, and it comes with press-along videos you follow from your chair. Unused tools can be returned within 30 days of delivery; email us first so we can approve it. No rush, though. The routine above works with your own hands.

When to see a doctor about lower back pain that wakes you at 3 a.m.

Most back pain that wakes you is not dangerous. Some is a message to see your doctor before you try anything at home. Please make that appointment if:

  • the pain is new, or getting worse week by week
  • it comes with a fever, or weight loss you can't explain
  • it is there even when you lie perfectly still, in every position
  • one or both legs feel weak, or numbness is spreading
  • you have trouble controlling your bladder or bowels, or numbness around the bottom — this one is urgent, call for help the same day

If you are pregnant, take blood thinners, or had back surgery recently, check with your doctor before pressing. For a medical view of back pain, read the NHS guide to back pain.

The honest timeline for lower back pain that wakes you at 3 a.m.

You may feel a spot ease within a few minutes of pressing it. That is the first layer.

The second layer is slower. Most people who press every evening notice a change in the second or third week, not the second night. About ten to fifteen minutes a night, for two to four weeks.

A 2021 review of clinical trials found encouraging signs for acupressure and low back pain, but the studies were limited in quality. Some people notice very little. I would rather tell you that now.

What This Won't Do

This won't stop your back from waking you on every night. It won't replace anything your doctor has given you. Keep taking exactly what was prescribed; this is something you do in addition.

What it gives you is ten minutes of something you can do yourself, before the lamp goes off. More pages for a sore back are collected on back pain relief.

Questions people ask about lower back pain that wakes you at 3 a.m.

What can I press when lower back pain wakes me at 3 a.m.? Nothing, at 3 a.m. itself. Change position, breathe out slowly and keep warm. Press before bed instead: the back of the knee, the outer ankle, the inner edge of the foot and the outer edge of the hand.

Which pressure point is used for lower back pain? The middle of the crease behind the knee, Weizhong (BL40), is the one most often used. Light pressure only, one to three minutes per leg, before bed.

What is the best acupressure tool for lower back pain? For your own lower back, a round-ended stick for the foot and ankle, plus a stick that sits in your palm. Avoid sharp tips. Choose something you can hold with your whole hand.

Is it safe to press if I take blood thinners? Ask your doctor first. Blood thinners make bruising easier, so use feather-light pressure and skip any spot that bruises.

How long before I notice a difference? Some people feel a spot ease within minutes. Steadier change, if it comes, usually takes two to four weeks of pressing each evening.

I have spent fifteen years studying traditional Vietnamese medicine, and I write about things you can do for yourself at home, sitting down.

This is general wellness education, not medical advice. It is not a substitute for care from your doctor. If you have a health condition, are pregnant, or take medication, check with your doctor first.
